To qualify for our senior care services, individuals typically need to meet the following criteria:
✔ Age Requirement – Must be at least 65 or old.
✔ Health & Care Needs – Require assistance with at least three activities of daily living (ADLs), such as bathing, grooming, dressing, or mobility.
✔ Risk for Nursing Home Placement – Must face a moderate risk of needing long-term nursing care or ongoing support to remain independent.
✔ Residency Requirement – Must reside within our service areas, including Boston, Middleboro, Raynham, Lakeville, Marion, Rochester, Easton, Plymouth, Wareham, Pembroke, Taunton, Freetown, Assonet, Mattapoisett, Tiverton, Swansea, Berkley, Norton, Dover, Wellesley, Dedham, and Needham.
✔ Financial Eligibility – Must meet Medicaid income and asset requirements or demonstrate financial ability to cover assisted living services.
